Patient Policies

New Patients

Please arrive 15 to 20 minutes prior to your appointment with your photo ID, insurance card, and list of medications you are currently taking.

Cancellations

Please call the office 24 hours in advance to cancel appointments. Failing to do so could result in a $25.00 charge, for which you are responsible, since insurance will not cover this assessment.

Late Policy

We are committed to prompt service, and will work very hard, barring emergencies, to stay on time. We may ask you to reschedule if you arrive for your appointment more than 15 minutes after your scheduled appointment. Always arrive 10 to 15 minutes early for an appointment to complete any paperwork associated with your visit.

Referral Authorizations

You should call the office at least three (3) days before the appointment to give us the information needed to complete the referral. Please submit the following information: your name, the physician you are being referred to, the phone and fax numbers, the date of the appointment, the diagnosis for the referral, your insurance, whether it is for a visit or a specific procedure (such as surgery), and where it is going to be performed (office or hospital). You should familiarize yourself with your insurance plan, and determine if it requires you to take a referral with you. 

Lab and X-Ray Results

Please contact Client Tell for your test results at 1-888-868-5236. Enter your medical records number, which is your SSN. Your test results are strickly confidental, only you can access them. If your results have not been posted on Client Tell within 10 business days please contact the office for results.

Medication Refills

Please contact your pharmacy 48 hours before needing your medication refilled.

Written Prescriptions or Forms to be Completed by Office

Written prescriptions and forms must be picked up at our office.

Patient Phone Calls

We will make every effort to return your phone call within 24 hours.  

Prior Authorization for Medications

Please allow 5 working days to acquire authorization for medications.  

Medical Record Request

Medical records are proceesed every Tuesday by Bactes, our copying service. Please complete the medical records release form and allow 7 to 14 business days for processing. For records that are being sent to a specialist please allow 48 hours for processing.

Emergencies and On Call

If you have an emergency, such as chest pain, severe shortness of breath, severe headache, or bleeding, call 911 or proceed directly to the nearest hospital emergency room. A physician is on call to answer your acute medical condition.  If the physician is not in the office, our on call service will pick up and page the provider when necessary.  If the doctor on call does not answer your call within 30 minutes, please recall the office.
